VIRTUAL
OFFICE TERMS OF SERVICE |
Monthly
rate for virtual office is $99.00 per month and is not
refundable. Your credit card will be charged on
monthly basis on the 1st of each month. Any fees
per month are not pro-rated if you cancel during the
billing period. The Virtual Office Package includes ten
(10) hours of conference room time per month and does
not carry over to the following month. Each additional
hour per month will be charged at the rate of $25.00 per
hour. Access is limited to normal business hours
from 8-5pm Monday-Friday. We are closed on
Holidays. This is a month to month agreement and either
party may cancel
at anytime.

AUTHORIZATION BY CLIENT |
I (we) hereby authorize Arizona Investors Group, LLC to initiate a monthly charge entry to my credit card
or ACH as listed above and to initiate adjustments (if necessary) for any transactions credited/debited in
error. This authority will remain in effect until Arizona Investors Group, LLC is notified by me in writing to cancel it in
such time as to afford Arizona Investors Group, LLC. and the financial Institution a reasonable opportunity to act on it.
All charges will be on the 1st day of each month.
